Head Smash deals damage, but the user receives 1⁄2 of the damage it inflicts in recoil. In other words, if the attack does 100 HP damage to the opponent, the user will lose 50 HP.
When a Pokémon is holding Rockium Z and uses its Z-Power, Head Smash turns into Continental Crush and has base power 200.
